Mechanism of action
Azathioprine is a prodrug of 6mercaptopurine (6MP). After intracellular conversion, its active metabolites inhibit de novo purine synthesis and are incorporated into DNA and RNA, blocking lymphocyte proliferation and impairing Tcell and Bcell function. It also modulates Rac1 signalling, contributing to immunosuppression.

Critical in immunology and transplantation research for modelling purine antimetabolite effects, Tcell regulation, autoimmune disease pathways and thiopurine pharmacogenomics (e.g., TPMT and NUDT15 polymorphisms).
Side effects and risks
Myelosuppression, hepatotoxicity, pancreatitis and heightened infection risk. Handling requires cytotoxic controls and genotoxic compound PPE.
