Mechanism of action
Alprostadil is a synthetic form of prostaglandin E1 (PGE1. It binds to specific G protein-coupled prostanoid receptors (primarily EP receptors) on vascular smooth muscle cells, activating adenylate cyclase and increasing intracellular cyclic AMP. This leads to relaxation of smooth muscle and potent vasodilation. In the corpus cavernosum, these effects promote increased arterial inflow and venous occlusion, facilitating erection. In the ductus arteriosus, alprostadil helps maintain patency in certain congenital heart defects.
Benefits and advantages
Alprostadil is widely used in erectile dysfunction therapy via intracavernosal injection, intraurethral administration or topical formulations, particularly when oral agents are unsuitable or ineffective. It is also a critical drug in neonatology to maintain ductus arteriosus patency pending corrective cardiac surgery. The molecule’s well-characterised pharmacology allows for precise, short-acting haemodynamic modulation.
Side effects and risks
In erectile dysfunction treatment, adverse effects may include penile pain, priapism, fibrosis at injection sites and hypotension. In systemic or neonatal use, potential risks are hypotension, flushing, apnoea and bradycardia, requiring close monitoring. As a potent vasoactive substance, alprostadil must be manufactured and handled under stringent conditions to ensure accurate dosing and to prevent accidental exposure.
