Mechanism of Action

Dimethyl fumarate (DMF) is an immunomodulatory compound that activates the Nrf2 antioxidant pathway and reduces NF-κB signalling. Its electrophilic properties allow covalent modification of KEAP1 cysteines, releasing Nrf2 and promoting transcription of detoxifying and cytoprotective genes. It also shifts Tcell populations toward antiinflammatory phenotypes.

Side Effects and Risks

Risks include lymphopenia, flushing, GI irritation, hepatotoxicity, PML risk (very rare) and cutaneous reactions. Handle with electrophilicimmunomodulator precautions.
