Mechanism of action
Ambuphylline is a xanthine-derived bronchodilator and respiratory stimulant. It inhibits phosphodiesterase enzymes, leading to increased intracellular cyclic AMP in bronchial smooth muscle, relaxation of the airways and improved airflow. It may also enhance diaphragmatic contractility and exert mild central nervous system stimulant effects.

Ambuphylline has been used in the management of obstructive airway diseases and as a component of combination products aimed at improving respiratory function. Its methylxanthine properties provide an alternative or adjunct to beta-agonists and inhaled therapies in selected clinical regimens.
Side effects and risks
Like other methylxanthines, ambuphylline has a narrow therapeutic window. Adverse effects include tachycardia, palpitations, insomnia, tremor, gastrointestinal irritation and, at high levels, arrhythmias or seizures. Therapeutic use requires careful dose control and monitoring for signs of toxicity. Handling of the API should be performed in facilities equipped to manage potent bronchodilators.
