Mechanism of action
Alpidem is an imidazopyridine-derived anxiolytic agent that acts as a high-affinity positive allosteric modulator at specific benzodiazepine sites on the GABAA receptor complex, with relative selectivity for the ω1 (BZ1) subtype. By enhancing the inhibitory action of GABA in brain regions involved in anxiety, it reduces neuronal excitability and exerts anxiolytic effects while aiming to minimise sedation compared with classical benzodiazepines.

Pharmacologically, alpidem was developed to provide potent anxiolysis with a lower risk of psychomotor impairment and dependence. Its receptor subtype selectivity and distinct chemical scaffold make it of continued interest in neuropsychopharmacology research as a prototype for non-benzodiazepine anxiolytics.
Side effects and risks
Clinical use of alpidem was limited by the emergence of serious hepatotoxicity, including cases of severe liver injury, which led to its withdrawal from many markets. Other adverse effects observed included sedation, dizziness, gastrointestinal symptoms and mood changes. Because of these safety concerns, alpidem is now mainly a research compound, and handling is restricted to controlled laboratory environments with appropriate protective measures and waste management.
