Mechanism of action
Alprenolol is a non-selective beta-adrenergic receptor antagonist that blocks both 1 and 2 receptors. By antagonising 1 receptors in the heart, it reduces heart rate, myocardial contractility and cardiac output, leading to decreased myocardial oxygen demand and lower blood pressure. 2 blockade in bronchial and vascular smooth muscle contributes additional haemodynamic and bronchial effects.
Benefits and advantages
Alprenolol has been used in the treatment of hypertension, angina pectoris and certain tachyarrhythmias. Its intrinsic sympathomimetic activity provides partial agonist effects at beta receptors, which may lessen resting bradycardia and negative inotropy in some patients compared with pure antagonists. The compound is also of value as a reference beta-blocker in cardiovascular pharmacology research.
Side effects and risks
Adverse effects typical of non-selective beta-blockers include fatigue, dizziness, cold extremities, bradycardia, hypotension and, in susceptible individuals, bronchospasm or exacerbation of peripheral vascular disease. Sudden withdrawal can precipitate rebound hypertension or angina. Beta-blockers may mask symptoms of hypoglycaemia in diabetic patients. Handling of the API requires measures to minimise dust inhalation and accidental exposure.
