Mechanism of action
Acetylcholine chloride is the quaternary ammonium ester of acetic acid and choline and is the prototypical endogenous neurotransmitter at muscarinic and nicotinic receptors. When administered pharmacologically, it stimulates cholinergic receptors in the autonomic nervous system, neuromuscular junction and central nervous system, producing a broad range of parasympathomimetic effects.

Because of its direct agonist activity at cholinergic receptors, acetylcholine chloride is primarily used in research and in specialised ophthalmic procedures to induce rapid miosis. It serves as an important reference compound in pharmacological assays investigating cholinergic transmission and receptor function.
Side effects and risks
Systemic administration produces profound parasympathetic stimulation with bradycardia, hypotension, bronchoconstriction, increased glandular secretion and gastrointestinal hypermotility, and is therefore not used therapeutically outside highly controlled settings. Even with local use, accidental systemic absorption can cause adverse cholinergic reactions. The API should be handled only in laboratories equipped to manage potent cholinergic agents.
