Doxycycline hyclate is a broad-spectrumtetracycline-classantibioticused in the treatment of infections caused bybacteriaand certainparasites.It is used to treatbacterial pneumonia,acne,chlamydia infections,Lyme disease,cholera,typhus, andsyphilis. It is also used to preventmalariaand in combination withquinine, to treat malaria. Doxycycline may be takenby mouthor byinjection into a vein. Common side effects includediarrhea, nausea, vomiting, and an increased risk ofsunburn. Use after the first trimester of pregnancy or in young childrenmay result in permanent discoloration of the teeth.Its use duringbreastfeeding is probably safe.Doxycycline is abroad-spectrum antibiotic, of thetetracycline class. Like other agents of this class, it either slows or kills bacteria by inhibiting protein production. It kills malaria by targeting a plastid organelle, the apicoplast. Doxycycline hyclate was patented in 1957 and came into commercial use in 1967.It is on theWorld Health Organization’s List of Essential Medicines. Doxycycline is available as a generic medicine. Medical use of Doxycycline hyclate

In addition to the general indications for all members of thetetracycline antibioticsgroup, doxycycline is frequently used to treatLyme disease, chronicprostatitis,sinusitis,pelvic inflammatory disease, acne,rosacea,andrickettsial infections. In Canada, in 2004, doxycycline was considered a first-line treatment forchlamydiaandnon-gonococcal urethritisand withcefiximefor uncomplicatedgonorrhea.

Antibacterial effects

Moraxella catarrhalis,Brucella melitensis,Chlamydia pneumoniae, andMycoplasma pneumoniaeare generally susceptible to doxycycline, while someHaemophilusspp.,Mycoplasma hominis, andPseudomonas aeruginosa have developed resistance to varying degrees. It is used in the treatment and prophylaxisofanthraxandLeptospirosis. It is also effective againstYersinia pestis(the infectious agent ofbubonic plague), and is prescribed for the treatment ofLyme disease, ehrlichiosis, andRocky Mountain spotted fever. Doxycycline hyclate is indicated for treatment of:

  • Rocky Mountain spotted fever,typhusfever and the typhus group,Q fever, rickettsialpox, and tick fevers caused byRickettsia
  • Respiratory tract infections caused byMycoplasma pneumoniae
  • Lymphogranuloma venereum, trachoma, inclusion conjunctivitis, and uncomplicated urethral, endocervical, or rectal infections in adults caused byChlamydia trachomatis
  • Psittacosis
  • Nongonococcal urethritis caused byUreaplasma urealyticum
  • Relapsing feverdue toBorrelia recurrentis
  • Chancroidcaused byHaemophilus ducreyi
  • Plague due toYersinia pestis
  • Tularemia
  • Cholera
  • Campylobacter fetusinfections
  • Brucellosis caused byBrucellaspecies (in conjunction with streptomycin)
  • Bartonellosis
  • Granuloma inguinale(Klebsiellaspecies)
  • Lyme disease

When bacteriologic testing indicates appropriate susceptibility to the drug, doxycycline may be used to treat these infections caused byGram-negative bacteria:

  • Escherichia coliinfections
  • Enterobacter aerogenes(formerlyAerobacter aerogenes) infections
  • Shigellaspecies infections
  • Acinetobacterspecies (formerlyMimaspecies andHerelleaspecies) infections
  • Respiratory tract infectionscaused byHaemophilus influenzae
  • Respiratory tract andurinary tract infectionscaused byKlebsiellaspecies

SomeGram-positive bacteriahave developed resistance to doxycycline. Up to 44% ofStreptococcus pyogenesand up to 74% ofS. faecalisspecimens have developed resistance to the tetracycline group of antibiotics. Up to 57% ofP.acnes strains developed resistance to doxycycline.When bacteriologic testing indicates appropriate susceptibility to the drug, doxycycline may be used to treat these infections caused byGram-positive bacteria:

  • Upper respiratory infections caused byStreptococcus pneumoniae(formerlyDiplococcus pneumoniae)
  • Skin and soft tissue infections caused byStaphylococcus aureus, includingmethicillin-resistantStaphylococcus aureusinfections
  • Anthrax caused byBacillus anthracisinfection

When penicillin is contraindicated, doxycycline can be used to treat:

  • Syphiliscaused byTreponema pallidum
  • Yawscaused byTreponema pertenue
  • Listeriosisdue toListeria monocytogenes
  • Vincent’s infection caused byFusobacterium fusiforme
  • Actinomycosiscaused byActinomyces israelii
  • Infections caused byClostridiumspecies

Doxycycline may also be used as adjunctive therapy for severeacne. The first-line treatment forbrucellosisis a combination of doxycycline andstreptomycinand the second-line is a combination of doxycycline andrifampicin(rifampin).

Antimalarial effects

Doxycycline is active against the erythrocytic stages ofPlasmodium falciparumbut not against the gametocytes ofPlasmodium falciparum. It is used to preventmalaria. It is not recommended alone for initial treatment of malaria, even when the parasite is doxycycline-sensitive, because the antimalarial effect of doxycycline is delayed. TheWorld Health Organization(WHO) guidelines state that the combination of doxycycline with either artesunate or quinine may be used for the treatment of uncomplicated malaria due toPlasmodium falciparum or following intravenous treatment of severe malaria.
