Mechanism of action
Carbimazole is a thionamide prodrug converted in vivo to the active metabolite methimazole. It inhibits thyroid peroxidase (TPO), the enzyme responsible for iodination of tyrosine residues on thyroglobulin and coupling reactions that form T₃ and T₄. This blocks synthesis of thyroid hormones without affecting their release, leading to gradual reduction of circulating thyroid levels.

Used in thyroid-pathophysiology research, Graves’ disease modelling, TPO-enzyme studies, autoimmune-thyroid disease investigations and comparative antithyroid SAR profiling. Side effects and risks
Risks include agranulocytosis, hepatotoxicity, rash, arthralgia and gastrointestinal upset. Careful handling and monitoring are required due to its potential for serious idiosyncratic reactions.
