Mechanism of action
Aminoglutethimide is an inhibitor of adrenal steroidogenesis and aromatase. It blocks the cholesterol side- chain cleavage enzyme (CYP11A1), reducing the synthesis of glucocorticoids, mineralocorticoids and sex steroids in the adrenal cortex, and also inhibits aromatase (CYP19), thereby decreasing the peripheral conversion of androgens to oestrogens.
Benefits and advantages
Aminoglutethimide has been used as an adrenolytic agent in the treatment of hormone-dependent breast cancer and Cushing’s syndrome, often in combination with glucocorticoid replacement to control excess adrenal hormone production. It is also of continuing interest as a research tool in endocrinology for dissecting steroid biosynthetic pathways.
Side effects and risks
Adverse effects include sedation, dizziness, skin rash, gastrointestinal disturbances, ataxia, hypothyroidism and, with prolonged use, adrenal insufficiency. Because it profoundly alters steroid hormone synthesis, treatment requires specialist supervision, hormone monitoring and appropriate replacement strategies. The raw material must be handled with great care to avoid occupational exposure, and manufacturing must follow stringent controls for potent endocrine-active substances.
