Mechanism of action

6Mercaptopurine (6MP) is a purine antimetabolite structurally related to hypoxanthine. It is converted intracellularly into thioinosine monophosphate and related metabolites that inhibit de novo purine synthesis and interfere with nucleic acid replication. By suppressing DNA and RNA synthesis in rapidly dividing cells, 6MP exerts cytotoxic and immunosuppressive effects.

6MP is widely used in protocols for acute lymphoblastic leukaemia and as a maintenance immunosuppressant in certain autoimmune and inflammatory conditions when formulated appropriately. Side effects and risks

Major risks include myelosuppression, hepatotoxicity and gastrointestinal toxicity. Genetic polymorphisms in thiopurine Smethyltransferase (TPMT) or NUDT15 can dramatically increase exposure to active metabolites and predispose to lifethreatening bonemarrow suppression. Other adverse effects include increased infection risk, pancreatitis and, with longterm use, potential secondary malignancies. Use requires specialist oversight, regular laboratory monitoring and pharmacogenetic consideration.
