Mechanism of action
Alpha-phytosterols are plant-derived sterols structurally similar to cholesterol. In the intestinal lumen they integrate into mixed micelles and competitively inhibit the incorporation of dietary and biliary cholesterol, reducing its uptake by enterocytes via the Niemann–Pick C1-like1 transporter. This results in decreased delivery of cholesterol to the liver and a compensatory increase in hepatic LDL receptor expression, thereby lowering circulating LDL cholesterol levels.
Benefits and advantages
Phytosterols are widely used as functional ingredients in foods and supplements aimed at cardiovascular risk reduction. When consumed in appropriate amounts, they can meaningfully lower LDL cholesterol as part of a heart-healthy lifestyle and may delay the need for pharmacological lipid-lowering therapy in some individuals. They are typically well tolerated and can be incorporated into a variety of formulations such as spreads, yoghurts and capsules.
Side effects and risks
Common side effects are usually mild and gastrointestinal in nature, such as bloating or changes in stool consistency. High intake of phytosterols may slightly reduce absorption of fat-soluble vitamins and carotenoids, which should be considered when designing long-term supplementation. Patients with rare sitosterolaemia, a genetic disorder of sterol transport, can accumulate phytosterols and are not suitable candidates for such products. As a raw material, alpha-phytosterol should be handled with normal precautions for lipid powders or granulates.
