Mechanism of action
Atracurium besylate is a non-depolarising neuromuscular blocking agent of the benzylisoquinolinium class. It competitively antagonises nicotinic acetylcholine receptors at the neuromuscular junction, preventing acetylcholine from triggering depolarisation and skeletal muscle contraction. Atracurium undergoes spontaneous Hofmann elimination and ester hydrolysis, processes that are largely organ-independent.
Benefits and advantages
Atracurium is a key compound in anaesthesiology research, neuromuscular physiology, receptor-binding assays, and pharmacokinetic modelling of organ-independent drug elimination. Its predictable degradation kinetics make it particularly useful in models of impaired hepatic or renal function.
Side effects and risks
Adverse effects include histamine release (flushing, hypotension, bronchospasm), prolonged neuromuscular block and, rarely, anaphylaxis. The breakdown product laudanosine may have CNS stimulant properties at high levels. Handling should follow high-potency neuromuscular-blocker precautions with strict avoidance of accidental exposure.
