Mechanism of action
Amcinonide is a high-potency topical corticosteroid. It binds to intracellular glucocorticoid receptors in keratinocytes and other skin cells, inducing transcription of anti-inflammatory proteins and suppressing the expression of pro-inflammatory cytokines, adhesion molecules and enzymes such as phospholipase A2. This results in reduced erythema, oedema, pruritus and overall inflammatory activity in the skin.
Benefits and advantages
Formulated as creams, ointments or lotions, amcinonide is used for short-term treatment of corticosteroid- responsive dermatoses, including eczema and psoriasis. Its high potency allows effective control of severe or recalcitrant lesions with relatively small amounts of drug, which can be advantageous in thick or lichenified plaques.
Side effects and risks
Local adverse effects include skin atrophy, striae, telangiectasia, purpura and delayed wound healing, especially with prolonged use, occlusion or application to delicate skin areas. Systemic absorption can lead to hypothalamic–pituitary–adrenal axis suppression, Cushingoid features, hyperglycaemia or growth retardation in children. Appropriate potency selection, treatment duration and application area are essential in clinical use. The API should be handled with care to avoid unintended dermal exposure.
