Mechanism of action
Amantadine hydrochloride is an adamantane derivative with dual pharmacological actions. As an antiviral agent, it blocks the M2 proton channel of influenza A virus, inhibiting uncoating of the viral particle within host cells. In the central nervous system it acts as a weak, non-competitive antagonist at NMDA-type glutamate receptors and enhances dopaminergic transmission, mechanisms believed to underlie its antiparkinsonian effects.
Benefits and advantages
Amantadine has been used in the treatment of Parkinson’s disease and drug-induced extrapyramidal symptoms, where it can improve rigidity, bradykinesia and gait disturbances. Historically it was also used for prophylaxis and treatment of influenza A infections, though resistance has limited this indication. Its unique pharmacological profile makes it valuable in neurology as an adjunctive therapy and in research on dopamine–glutamate interactions.
Side effects and risks
Common adverse effects include insomnia, dizziness, dry mouth, peripheral oedema, livedo reticularis and gastrointestinal upset. Neuropsychiatric reactions such as confusion, hallucinations and agitation can occur, particularly in elderly patients or at higher doses. Amantadine must be dose-adjusted in renal impairment to avoid accumulation and toxicity. The API should be handled using appropriate precautions to prevent accidental exposure.
