Mechanism of action
Altizide is a thiazide-type diuretic that acts primarily in the early distal convoluted tubule of the nephron. It inhibits the Na+/Cl− symporter, reducing sodium and chloride reabsorption and increasing their excretion along with water. The resulting reduction in plasma volume lowers cardiac preload and blood pressure. Chronic use also decreases peripheral vascular resistance through mechanisms that are not fully elucidated.
Benefits and advantages
Altizide is used in the management of hypertension and oedematous states, either alone or in combination with other antihypertensive agents. Thiazide-type diuretics are well established as effective, orally active drugs with a favourable impact on cardiovascular outcomes in appropriate patient populations. Altizide can be combined with potassium-sparing agents or other antihypertensives to tailor therapy.
Side effects and risks
Typical adverse effects include electrolyte disturbances (hypokalaemia, hyponatraemia, hypomagnesaemia), hyperuricaemia, hyperglycaemia and, less commonly, photosensitivity or gastrointestinal upset. In susceptible patients, thiazides may precipitate gout or worsen glucose tolerance. Clinical use requires periodic monitoring of electrolytes and renal function. The API should be handled using standard precautions for diuretic agents.
