Mechanism of action
Aceclidine hydrochloride is a direct‑acting muscarinic receptor agonist. When applied topically in the eye it stimulates the sphincter pupillae and ciliary muscles, causing miosis and increased aqueous humour outflow via the trabecular meshwork. This leads to reduced intra‑ocular pressure.
Benefits and advantages
Aceclidine has been used in the management of glaucoma and for producing miosis during ophthalmic procedures. Its local mechanism allows targeted reduction of intra‑ocular pressure without systemic beta‑blockade or carbonic anhydrase inhibition, making it an alternative option in specific clinical scenarios or combination therapy.
Side effects and risks
Local adverse effects may include ocular irritation, conjunctival hyperaemia, accommodative spasm and transient blurred vision. Systemic cholinergic effects such as headache, sweating or bradycardia are unlikely but possible if significant systemic absorption occurs. As an API, aceclidine should be handled under controlled conditions to avoid accidental exposure, particularly to the eyes.
