Mechanism of action
Abacavir is a nucleoside reverse transcriptase inhibitor (NRTI) used in antiretroviral therapy. After intracellular phosphorylation to carbovir triphosphate, it competes with endogenous deoxyguanosine triphosphate for incorporation into viral DNA by HIV1 reverse transcriptase. Chain termination of the growing viral DNA strand results in inhibition of viral replication.

The most critical risk is a potentially lifethreatening hypersensitivity reaction strongly associated with the HLAB*57:01 allele. Other adverse effects include gastrointestinal symptoms, fatigue, rash and a possible increase in cardiovascular risk in susceptible populations. Handling and formulation of abacavir should be restricted to qualified professionals working under appropriate safety regulations.
