Mechanism of action
Aminophylline is a 2:1 molecular complex of theophylline and ethylenediamine, designed to improve the solubility and parenteral usability of theophylline. Theophylline inhibits several phosphodiesterase (PDE) isoenzymes, leading to accumulation of cyclic AMP in airway smooth muscle cells, thereby enhancing relaxation and bronchodilation. Additional mechanisms include adenosine receptor antagonism, mild anti-inflammatory actions and increased diaphragmatic contractility.
Benefits and advantages
Aminophylline allows consistent intravenous dosing in acute bronchospasm, neonatal apnoea and emergent respiratory failure. It is commonly used in research addressing airway signalling pathways, PDE biochemistry, respiratory drive modulation and systemic methylxanthine pharmacokinetics.
Side effects and risks
Its narrow therapeutic index requires vigilance. Toxicity may manifest as severe tachyarrhythmias, seizures, hypotension, persistent vomiting and metabolic acidosis. Plasma concentration monitoring is standard in clinical use. The API must be handled in ventilated environments due to CNS-active potency.
