Mechanism of action
Aloxiprin is a complex of acetylsalicylic acid (aspirin) with glycine. Once administered, it is gradually hydrolysed in the gastrointestinal tract, releasing aspirin, which irreversibly acetylates cyclo-oxygenase enzymes (COX-1 and, to a lesser extent, COX-2). This blocks the conversion of arachidonic acid to prostaglandins and thromboxanes, producing anti-inflammatory, analgesic, antipyretic and antiplatelet effects. The glycine component acts as a buffering agent that can improve gastric tolerability compared with plain aspirin.
Benefits and advantages
Aloxiprin offers the well-known benefits of aspirin with potentially reduced gastric irritation in some patients. It can be formulated for the management of mild-to-moderate pain, fever and inflammatory disorders, or for low-dose antiplatelet regimens where appropriate. The buffered complex may be advantageous in individuals who require salicylate therapy but are sensitive to direct gastric acid exposure.
Side effects and risks
Adverse effects are similar to those of other salicylates and include dyspepsia, gastrointestinal ulceration, bleeding, bronchospasm in aspirin-sensitive individuals, renal impairment and, in overdose, salicylate toxicity. Concomitant use with other anticoagulants or NSAIDs increases bleeding risk. As a raw material, aloxiprin should be handled with care to avoid inhalation or prolonged skin contact, and stored under conditions that prevent hydrolysis and degradation.
