Mechanism of action

Acetylsulfisoxazole is an acetylated sulfonamide antibacterial. After deacetylation in vivo it acts as a competitive inhibitor of dihydropteroate synthase in the folate synthesis pathway of susceptible bacteria, thereby inhibiting growth through impairment of nucleic acid synthesis.

Benefits and advantages

Sulfonamides have broad antibacterial activity and have historically been used for urinary and systemic infections. The acetylated derivative improves certain physicochemical properties, including solubility and tolerability, and can be incorporated into combination formulations.

Side effects and risks

Adverse reactions include hypersensitivity reactions (from mild rash to severe StevensJohnson syndrome), haematological disturbances, crystalluria and, rarely, hepatotoxicity. Crossreactivity with other sulfonamide drugs is possible. Use in humans requires careful risk assessment and monitoring; industrial handling must respect hazardoussubstance regulations.
